Founded by Dr Jeremy Rawlins
Dr Jeremy Rawlins founded Forme Specialist Plastic Surgery in 2024, after having been in private practice for many years. He has lived and worked in the UK, USA, and now Australia, gaining a deep understanding of the diverse populations and their surgical needs.
Jeremy is a well-known plastic surgeon both in Australia and internationally, recognised for his research, clinical skills, teamwork, and leadership. He’s published hundreds of papers on Plastic and Reconstructive Surgery and has edited various academic materials. In 2023, he released a textbook titled “Core Techniques in Flap Reconstructive Microsurgery.”
He works in both public and private healthcare in Perth, delivering high-quality care. At Royal Perth Hospital, his job involves handling complex cases of trauma and cancer, often using advanced microsurgery. He also leads a team at the State Adult Burns Unit at Fiona Stanley Hospital in Murdoch. Additionally, Jeremy has been setting up a burn centre in Moshi, Tanzania, for the last ten years.
When he’s not working, Jeremy enjoys water sports like boating, sailing, windsurfing, and swimming in Western Australia’s waters. He also keeps fit by regularly going to the gym or by running and cycling.

My secret to endless energy? I have a varied practice that keeps me excited and motivated in everything I do.
My background? I’ve had the good fortune to live and work in Australia, the UK, and the USA. It’s tough to choose a favourite—they each have their unique charms.
My next travel destination? I’m lucky to be deeply involved in plastic surgery, and I often get invited to speak at international conferences. It’s a privilege to share what I know.
My favourite type of surgery? There are too many to name, but I have favourites in each subspecialty I work in. Just ask me about them!
Keeping fit at my age? It’s crucial to stay active. My routine includes daily gym sessions with stretching and weight training, cardio like swimming, cycling, running, and eating healthily.
Enjoying a drink? Only in moderation. We live near some of the finest vineyards in the world. My top pick? A good WA Cab Merlot.
Listening to music while operating? Yes, I love all kinds of music—from Pop and Rock to Soul and Techno. I even DJ a bit. And yes, I love singing too.
Relaxing when not working? I travel, boat, and enjoy all sorts of outdoor activities with family and friends. I just don’t know how to stay still!

Where will my surgical procedure be performed?
Many local anaesthetic procedures can be performed in our beautiful and well-appointed clinic in Murdoch. For surgery requiring general anaesthetic, sedation, and /or overnight stay in hospital, your procedure will be performed in one of the private hospitals in the Perth metropolitan area.
Our surgeons are accredited at St John of God Hospitals (Murdoch, Mount Lawley, and Subiaco), Hollywood Private Hospital, and the Park Private Hospital.
What changes have been implemented by AHPRA regarding cosmetic surgery as of July 1, 2023?
From 1 July 2023, changes to cosmetic surgery have been put in place by AHPRA to ensure their safety and well-being. Patients considering cosmetic surgery will be required to follow the below process.
GP Referral
Patients must obtain a referral from a general practitioner (GP), preferably their usual GP, before attending their first pre-operative consultation with their surgeon. GP referrals are valid for 12 months and Specialist referrals for 3 months. The referral starts from the date of your first consultation.
Pre-operative Consultations
Patients are required to have a minimum of two pre-operative consultations, with at least one consultation conducted in person. The surgeon will discuss what the surgery involves, the risks and possible complications, the recovery period and any restrictions or additional treatment you will require. You will be provided by the clinic the total costs, including details of deposits required and payment dates, refund of deposits, payments for follow-up care and possible further costs for revision surgery or additional treatment.
Psychological Assessment
Patients are required to undergo an assessment to evaluate any underlying psychological conditions such as Body Dysmorphic Disorder. This assessment helps to ensure that patients are suitable candidates for surgery.
Cooling-Off Period:
There must be a cooling-off period of at least seven days after the patient has had two consultations and given informed consent (signed consent forms), before the surgery can be booked or a deposit paid.
What are the cosmetic consultation fees?
The first consultation will cost $300.00, we ask patients to make a 50% deposit to secure their appointment at the time of booking the consultation, remaining costs are payable on the day of consultation. The consultation will typically take 30-60 minutes depending on the procedure you are interested in. If a second consultation is required, this will cost approximately $150. If a Medicare item number applies to your potential procedure, then you will be entitled to a rebate from Medicare.
What are the consultation fees for medical necessary procedures (skin cancers, reconstructive surgery and hand traumas)?
The consultation fee is $300 for first consult and $150 for any follow up consults. All consult fees are payable in full on the day of your consultation.
Medicare rebates are applicable, providing you have a valid referral from your GP or Specialist.
All patients will be required to complete a patient form prior to your consultation taking place.

Forme Specialist Plastic Surgery are committed to providing our patients with the highest quality of care and recognise the right of our patients, family and carers, to make a complaint about the care and service they receive from our practice. Our complaints policy aims to be accessible and simple to understand to ensure ease of use. All patients will be given a copy of this policy on request.
